School Nurse Assistant Program (SNAP) Parents Connection to Their Child's School Health and Wellness Information
This web-based program for Liberty families provides an easy, secure, and paperless way for parents to update and review school health information and medical documents for their children throughout the school year.

Fall vision and hearing screenings are mandated for all incoming students and current Pre-Kindergarten, Kindergarten, 1st, 3rd, 5th, and 7th grade students. In the spring, students will be assessed for Acanthosis Nigricans, a pre-diabetes skin marker, and for Scoliosis.
New Medication Policy
To take medication while at school a student must have a medical necessity and both parents will need to complete the Medical Authorization Request (MAR) Form found on the right side of this page.
All Non‐Prescription medications (including Cough Drops) must be in their original container if provided from home. The Medication Administration Request Form must be completed with both parents or guardians signatures. All Non‐Prescription medications to be administered for longer than 10 days or given more than 4 times in a month must also have this written request signed and dated by a prescribing physician.
All Prescription medications must be in their original labeled packaging. The label must include: the student’s name, the physician’s name, the name and strength of the drug, the amount of drug to be given, frequency of administration, and date prescription was filled. All Prescription medications must be accompanied by a Medication Administration Request Form signed and dated by the prescribing physician and both parents or guardians requesting this service.
