School Nurse Assistant Program (SNAP) Parents Connection to Their Child's School Health and Wellness Information
This web-based program for Liberty families provides an easy, secure, and paperless way for parents to update and review school health information and medical documents for their children throughout the school year.

Review and update new health information such as vaccinations, health conditions, and screening results.
Receive immediate notifications when there are changes in your child’s health status.
Easily message the Nurses Office at Liberty to expedite communication.
Fall vision and hearing screenings are mandated for all incoming students and current Pre-Kindergarten, Kindergarten, 1st, 3rd, 5th, and 7th grade students. In the spring, students will be assessed for Acanthosis Nigricans, a pre-diabetes skin marker, and for Scoliosis.
To take medication while at school a student must have a medical necessity and the parent will need to complete the Medical Authorization Form found on the right side of this page.
Non prescription medicine must also be in the original container with the original label and the student's name. Liberty will not accept medications in unlabeled bottles, baggies, etc.
